Promotional Activities Request Form Guidelines:
1. Due to the number of requests received, Minnesota State University Athletics will only honor one promotional activities request per requesting agency/individual each academic year.
2. The Promotional Activities Request Form must be completed in its entirety, printed off, then signed by the requestor. All forms must then be sent to:

Filling out and sending in the Promotional Activities Request Form for does NOT guarantee your request will be filled. All questions can be directed to one of the above two individuals. Please allow at least two (2) weeks for all Promotional Activities Request responses. MSU Athletics will do its best to promptly respond to all requests within two (2) weeks upon receipt of the request and appreciates your patience and understanding in this matter.
